This project designs and implements development of a Disease Surveillance Case Count Reporting System for epidemiology units. The system was developed using PHP, MySQL, HTML/CSS, JavaScript. It provides modules for case definitions, daily counts, thresholds and alerting. Testing shows the system improves record keeping, reduces paperwork and supports timely decisions.
